Who this policy covers

This policy describes how Center for Compassion, PLLC ("we", "us") handles information collected through thecenterforcompassion.com.

It does not describe how we handle protected health information for people who are clients of the practice. That is covered by our HIPAA Notice of Privacy Practices, which you receive as part of your intake paperwork. Completing a form on this website does not make you a client of Center for Compassion, PLLC.

Information you give us

When you submit our booking form, we collect the information you type into it: your name, email address, phone number, age, pronouns if you provide them, a description of what you are seeking support around, your scheduling availability, your insurance carrier, whether you live in Arizona, your preferred clinician, and how you heard about us.

We use this information for one purpose: to contact you about your request and to match you with an appropriate provider. We do not sell it, rent it, or use it for advertising.

How that information reaches us

Booking form submissions are delivered to our practice inbox by email. Standard email is not an encrypted or HIPAA-secure channel. This is why our form asks you to acknowledge that the information you provide cannot be guaranteed to be kept confidential, and why we encourage you to share only what feels comfortable at this stage.

If you would prefer not to send details in writing, call or text us at 480-530-6441 instead.

Text messages

If you check the SMS opt-in box, you agree to receive text messages from us regarding your inquiry, sent via an automated system. Your number will not be used for marketing. Message and data rates may apply. You can opt out at any time by replying STOP or telling us directly.

Information collected automatically

Our hosting provider records standard server request data such as IP address, browser type, and pages requested, for security and reliability purposes.

This site does not use advertising trackers or third-party analytics cookies. Our typefaces are served from our own domain, so no font provider sees your visit. One third-party resource is loaded: an embedded Google Map on our home page, which is subject to Google's own privacy policy and only loads if you scroll to it.

How long we keep it

Booking request emails are retained in our practice inbox according to our internal records policy. If you would like your inquiry deleted and you did not become a client, email info@thecenterforcompassion.com and we will remove it.

Your choices

You may request a copy of the inquiry information we hold about you, ask us to correct it, or ask us to delete it. Contact info@thecenterforcompassion.com or call 480-530-6441.

If you are a client of the practice, your rights regarding your health record are described in our HIPAA Notice of Privacy Practices and are broader than what is described here.

Children

This website is not directed to children under 13, and we do not knowingly collect information from them through it. Minors receiving services at Center for Compassion are enrolled through a parent or guardian.

Changes to this policy

We may update this policy from time to time. The effective date at the top of this page reflects the most recent revision.
